Your initial appointment will not be like most first visits at a dental office. It starts with a friendly greeting from our office manager, Heidi. Unlike corporate dental practices, we don’t have a revolving door for staff. Our team members are members of our dental family, and the lasting relationships we build start in our house and extend to all our patients.
Heidi is great at helping you feel comfortable and welcomed at our office, whether it’s your first visit or your 100th.
Before Your First Visit
To help everyone stay on time, please assist us by assembling the following information for your initial visit:
- Your referral slip and any x-rays (if applicable)
- A list of your current medications
- If you have medical or dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.
- Please alert the office if you have a medical condition that may be of concern before surgery. (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.)
In addition, please fill out the new patient forms online. If you are having difficulty, you can call us for assistance or attend your appointment at least 15 minutes early. Then, Heidi or someone else in our office will help you complete the forms before your appointment.
Your Initial Exam
Dr. Bob Fornalczyk and a team member will see you. You’ll get to meet them and talk about your previous dental experiences and your goal for current treatment. Whether you’re looking to maintain your oral health or achieve the smile of your dreams can significantly affect how we plan and execute your dental care. We may ask additional questions to make sure we fully understand your goals.
You should also let us know if you have any restrictions that can affect your dental treatment, such as budget restrictions, a date you need results by, or procedures you’d like to avoid, such as surgery or braces.
Then, Dr. Bob and the team members will do a complete and thorough examination. The exam will consist of several different assessments:
- Charting of all existing dental work, possible cavities, tooth fractures, and missing teeth.
- Periodontal charting to check for gum disease and determine the health of your gums.
- Oral cancer screening is done with a VELscope.
- A full mouth set of X-rays taken digitally.
- Intraoral photos taken with a digital camera.
- A complimentary digital smile makeover with LUMISmile.
We may ask additional questions based on the results of your exam. Based on the exam and your treatment goals, we’ll schedule your next appointment, which might be a cleaning and checkup in up to six months or a detailed consultation in the next few weeks.
Free Initial Consultation
The doctor will use the data from your initial exam to create a treatment plan specific to your needs and review it with you at a free consultation.
During this consultation, you’ll discuss treatment options for your current oral health and treatment goals. You might discuss:
- Cosmetic dentistry options
- Restorative dentistry options
- Dental implant options if you’re missing teeth
Modern dentistry typically gives us multiple options to achieve the results you’re looking for. We’ll present these options to you and explain the benefits and limitations of each option. You are always in control of your dental care, and the final decision of what treatment is best for you is in your hands. Often, this includes the option to do nothing but simply wait and see.
Once we decide on a plan of action, we’ll schedule your first treatment visit.
IMPORTANT: A parent or guardian must accompany all patients under 18 at the consultation visit.
